​1. Look on the back of your insurance card to get the number to member services.
​2. Call the member services number and ask if they recognize: Calvin Smith, MA, LPC as an in network provider
​3. If they do not, ask if they accept out of network providers for your insurance plan.
​4. Once you have determined the kind of coverage your insurance provides for our services,
